Community nursing is when nursing expertise and medical care is provided right in your own home, without having to go to a hospital.
Nurses help with things like managing your medication, changing bandages, doing health assessments or even giving you tips to stay healthy. This way, you can recover or maintain your health from the comfort of your own home, surrounded by familiar things.
How does it work?

If eligible, you'll be connected with our community nursing team who will assist you every step of the way.
Our nursing team will visit you and complete a comprehensive assessment. The assessment will help us to identify your individual needs for care and develop a personalised care plan.
We will also work with you to explore long-term care options so you have a plan in place for the future, allowing you to focus on your recovery with peace of mind.
