Our network of nurses, allied health practitioners and support workers work with your general practitioner to ensure that the care you receive helps with every step of your recovery journey.

What is the Hospital to Home Transition Program?
The hospital to home transition program helps people who have been in the hospital to get back on their feet at home. Trinity Support Australia offers short-term support, like therapy, nursing help, and personal care, so you can regain your independence and recover at home while long-term care options are put in place.
How does it work?
Our team will visit you in the hospital or contact you / your representative to understand your needs and eligibility. You can request this assessment by talking to your hospital staff or contacting us at hi@trinitysupportaustralia.com.au
If eligible, you'll be connected with our transition care team who will assist you every step of the way.
Our team will help with a smooth discharge from the hospital and provide short-term support at home, including therapy, nursing assistance, and personal care.
Our team will also work with you to explore long-term care options so you have a plan in place for the future, allowing you to focus on your recovery with peace of mind.

We offer two convenient options for receiving care:
In Your Own Home - For low care needs, our team will provide supports at home or out in the community.
At Our Private Facility - For high care needs, we offer support in our own private residential home. This option provides access to 24/7 support.

Nursing care: assist with medication management, wound care, and other medical needs.
Personal care: support with personal hygiene tasks, such as toileting you to focus on your recovery.
Domestic assistance: help with household chores, ensuring a safe and comfortable home environment.
Occupational therapy: assist with daily living activities, such as participating in hobbies and interests, leisure activities and skills training.
