Overview
What is Supported Independent Living (SIL)?
The hospital to home transition program helps people who have been in the hospital to get back on their feet at home. Trinity Support Australia offers short-term support, like therapy, nursing help, and personal care, so you can regain your independence and recover at home while long-term care options are put in place.
How does it work?

If you are eligible for SIL, you'll be connected with our team who will assist you every step of the way.
Our team will complete an assessment to help us to identify your individual needs and preferences. We will also assess if SIL might be a good fit for you.
Once you move in, Trinity provides support with personal care, medication management, meals and skills/capacity building. During your stay with us, we conduct regular reviews to ensure that your goals are being met and adjusted as needed.

In Your Own Home - Our team can take over the management of your home and provide supports within your home
At Our Private Facility in Adelaide, South Australia - Alternatively, we provide support at our own private residential facility in Adelaide. This option provides access to 24/7 support.

Daily Living Assistance
Personal care: includes help with tasks like showering, dressing, toileting, and managing hygiene routines.
Meal preparation and assistance: we can help with grocery shopping, cooking meals, and ensuring dietary needs are met.
Light housekeeping: involves assistance with tasks like laundry, vacuuming, and maintaining a clean living space.
Medication management: we can give you or remind you to take medications and ensure proper dosage is taken.
Additional Support
Community access and transportation: help with getting to appointments, social events, and participating in community activities.
Social skills development: we can help individuals build communication and social interaction skills.
Budgeting and financial management: help with managing finances, including paying bills and tracking expenses.
Life skills development: can include learning new skills like cooking, using public transportation, or managing household chores independently.
